Changes in systolic (SBP) (A) and diastolic blood pressure (DBP) (B) control rates at six months compared to baseline across baseline patients with uncontrolled SBP (A) or DBP (B), respectively. There were no statistical differences in changes in these cardiometabolic control rates between the intervention and control groups with the exception of SBP in aging patients where the intervention group had 11% higher SBP control rates as compared to the control group.
