Principal cell in cortical collecting duct. Increased epithelial sodium channel (ENaC) as in Liddle syndrome, and hyperaldosteronism, either primary or secondary, may cause hypokalemic alkalosis with hypertension. Decreased ENaC, mineralocorticoid receptor blockade, and type 4 renal tubular acidosis resulting from hypoaldosteronism or aldosterone resistance present with hyperkalemic acidosis with hypotension.
