Advantages
 More ergonomic hand motion for Achilles tendon debridement
 Easier access to ventral surface of Achilles tendon
 Better orientation of inside structures
 Concomitant chondral lesions of ankle can be dealt with arthroscopically
Risks
 Injury to sural nerve
 Injury to calcaneal branch of tibial nerve
 Injury to Achilles tendon insertion
