Predictive margin adjusted for age and its square, sex, index year, index month, total covered charges from outpatient, inpatient and carrier files in the year preceding index fistula/graft creation date, inpatient setting for the index fistula/graft creation and all co-morbidities available from the Chronic Condition Data Warehouse (except chronic kidney disease), community contextual factors and hospital referral region.
