Models 1b, 1c, 2b, 2c, 3b, 3c have the same covariates, distance specification and choice set definitions as in Table 2. Distance coefficient is ${\widehat{\mathit{\beta }}}_d+2{\widehat{\mathit{\beta }}}_{d^2}\overline{d}+3{\widehat{\mathit{\beta }}}_{d^3}\overline{d^2}$ where $\overline{d}$ and $\overline{d^2}$ are computed as the mean of the LSOA centroid to practice distance and squared distance for the whole sample. PA: population achievement is ${\sum }_q\left(N_{jq}/\left(D_{jq}+E_{jq}\right)\right)({\mathit{\pi }}_q^{max})/({\sum }_q{\mathit{\pi }}_q^{max})$ where N _jq is the number of patients for whom QOF clinical indicator q is achieved by practice j, D _jq is the number of patients who are declared eligible for indicator q, E _jq is the number of patients exception reported for indicator q by practice j. ${\mathit{\pi }}_q^{max}$ is the maximum number of points achievable for indicator q. RA: reported achievement for practice j is ${\sum }_q(N_{jq}/D_{jq})({\mathit{\pi }}_q^{max})/({\sum }_q{\mathit{\pi }}_q^{max})$ ACSCs is the number of emergency admissions for ambulatory care sensitive conditions of practice patients per 10,000 patients registered with the practice.
