EpCAM expression in normal mucosa is commonly restricted to cells of the suprabasal layers. During tumor formation through sequential mutations, EpCAM expression is frequently increased in cells of primary carcinomas. Circulating and disseminated tumor cells (CTCs/DTCs) display mixed expression patterns with retained or lost expression of EpCAM. Macrometastases is often characterized by strong expression of EpCAM, which is similar to the corresponding primary tumor.
