A girl with a primary triple structural curve (left proximal 31°, right main thoracic 26°, and left lumbar 26°) (b), was fitted with a first prototype including “D modifier” proximal pad in combination with a removable compression-traction superstructure (a). The in-brace X-ray showed a mild correction of all the curves (c). Spinal balance was significantly improved with this brace, and proximal curve did not increased but decreased also significantly. After observing that the apical region of the proximal curve was relatively low (disc T4–5), it was decided to design a complex brace, which included a third “three-point system” still working underarm (d) (no X-ray available). This exclusive design formed by three underarm “three-point systems” had not been used ever before, to our knowledge, and helped to stabilize this scoliosis. The risk of failure is the highest for this curve pattern, especially in this particular case where the proximal curve is the major and most rigid one
