Adults with mental illness who have not been adherent with treatment and have had: (a) ≥2 psychiatric hospitalizations in the past 36 months or any forensic mental health treatment in prior 6 months; and (b) one of more acts of serious violent behavior toward self or others or threats of, or attempts at, serious physical harm to self or others within the last 48 months |
Individual treatment plans must include either ACT or case management services |

Trained case manager helps individuals access needed medical, social, psychosocial, educational, financial, and other services required to encourage independent functioning in the community |
