Source: Centers for Disease Control and Prevention (CDC) WONDER, 2007–2009. Data provided by personal communication with the National Heart, Lung, and Blood Institute. The Agency for Healthcare Research and Quality has released state-level data for heart disease for all 50 states and the District of Columbia; the data are taken from the congressionally mandated National Healthcare Quality Report.49 In addition, the Women’s Health and Mortality Chartbook of the National Center for Health Statistics has state-related data for women.50 Metropolitan/micropolitan area risk data are available for 500 such areas nationwide.51 Behavioral Risk Factor Surveillance System data are also collected within each state.52 The CDC has the Geographic Information Systems, which provides mortality rates down to the county level, by sex and ethnicity.53 The 2008 Atlas of Stroke Hospitalizations Among Medicare Beneficiaries is a new resource that provides data down to the county level, by sex and race.54
