This measure was introduced with the following verbal introduction (English version): Now I’m going to ask you some questions about how decisions were made with your mental health care or substance abuse provider during the last visit you had with your provider. If you didn’t see your provider today, think back to the last appointment you had with your provider. For each statement, please indicate how much you agree or disagree.”
