Survival estimates are derived from Cox models that include covariates of age, gender, race/ethnicity, discharge year, primary discharge diagnosis of sepsis, acute stroke, hematologic malignancy, or solid malignancy, mechanical ventilation use and duration, hospital length of stay, Charlson comorbidity index, Do-Not-Resuscitate order, and neighborhood median household income, and strata for site of hospitalization, admission from a skilled-care facility, and discharge to a skilled-care facility. P-Values are for likelihood ratio tests comparing a full model to one without the categorical variable of insurance coverage.
