  • Using a contraceptive method other than withdrawal or natural family planning AND age 30–44 years OR

  • trying to conceive for ≥12 months OR

  • trying to conceive for 5–11 months AND age 35–49 years OR

  • (not trying to conceive OR don't know) AND (not surgically sterile OR menopausal) AND (age 45–49 years OR had not had sex with a male in the past 3 months OR partner had a vasectomy)

Moderate
  • Using a contraceptive method other than withdrawal or natural family planning AND age 18–29 years OR

  • trying to conceive for 5–11 months AND age 18–34 years OR

  • had sex with a male within 3 months, age 35–44 years AND (not using any contraception OR using withdrawal OR natural family planning)

High non-tryer Had sex with a male within 3 months, age 18–34 years and (not using any contraception OR using withdrawal OR natural family planning)
High tryer Trying to conceive for <5 months
