For the posteromedial approach, the skin incision is performed along the medial head of the gastrocnemius. After blunt preparation, the semi-membranosus complex can be retracted medially and the medial gastrocnemius can be retracted laterally. After subperiostal dissection of the popliteus muscle, the posteromedial tibia can be approached. Reprinted from Galla M, Riemer C, Lobenhoffer P.
