Sagittal unenhanced proton density-weighted 1.0T magnetic resonance imaging (MRI) shows diffuse full thickness cartilage loss at the central subregions of the medial femur (arrows) and tibia (arrowhead). Note anterior and posterior tibial and femoral osteophytes. In addition there is partial maceration of the posterior horn of the medial meniscus. B. Axial contrast-enhanced T1-weighted MRI of the same knee (acquired on the same day) shows moderate synovial thickening and enhancement at the medial and lateral patellar recesses (arrows). In addition there is joint effusion depicted as hypointensity (unenhanced) within the medial synovitis.
