*NTM, nontuberculous mycobacteria; TB, tuberculosis. †Minimal: lesions that are slight to moderate density or bronchiectasis without cavitation. Lesions may be present in a small portion of 1 or both lungs, but the total extent of the lesions should not exceed the volume of lung on 1 side, which is present above the second chondrosternal junction (or apex of 1 lung). Moderate: lesions may be present in 1 or both lungs, but the total extent must not be more than the following: 1) scattered lesions of slight to moderate density or bronchiectasis that may extend throughout the total volume of 1 lung or the equivalent volume in both lungs; 2) dense, confluent lesions that are limited in extend to one third of the volume of 1 lung; 3) cavitation with a diameter <4 cm. Advanced: lesions that are more extensive than moderately advanced.
