Key examination information, including accession number, modality, and site of examination, are automatically passed from PACS into form, reducing amount of radiologist data entry. Radiologist defines issue category by selecting one of checkboxes and then adds comment to clarify issue. Final issue category is determined by quality assurance team member after issue resolution.
