Notes: NCQA = National Committee for Quality Assurance; PCMH = patient-centered medical home. PCMH capabilities were assessed using NCQA’s 2011 application for PCMH recognition, which awards points for meeting each of NCQA’s recognition standards. We used point totals to define four levels of PCMH capabilities equivalent to each of NCQA’s PCMH recognition levels (or no recognition).
