In this paper by Lee and colleagues (BMJ 2017;358:j3677, doi:10.1136/bmj.j3677), we incorrectly provided India as the country in the second affiliation of one of the authors, Zulfiqar A Bhutta. Karachi is, of course, in Pakistan.
