Accessory papillary muscle that arises from the ventricular septum and is attached to the side of the anterior leaflet. This accessory muscle is excised in its entirety; the fibrous attachments to the side of the leaflet are cut and the papillary muscle is amputated at its base and removed (dotted line). Chordal attachments to the leading edge of the anterior leaflet are preserved.
