A potential follow-up pathway for patients after an AKI episode. This pathway shows different recommended strategies for patient monitoring after an AKI episode. The intensity of monitoring depends on the patient’s risk of adverse events (Figure 2, to be assessed at the patient’s last point of contact with the health care system) and the results of the initial follow-up visit. Follow-up should also be individualized (e.g., moderate-risk patients could be treated as low risk or high risk) and dynamic. For example, patients thought to be low risk may require more frequent, nephrologist-led monitoring if they are found to have evidence of CKD or CVD at their initial post-AKI visit.
