Body mass index categories were normal weight, ≥18.5 kg/m2 and <25 kg/m2; overweight, ≥25 kg/m2 and <30 kg/m2; class I obesity, ≥30 kg/m2 and <35 kg/m2; class II obesity, ≥35 kg/m2 and <40 kg/m2; and class III obesity, ≥40 kg/m2. Hypertension was defined as systolic blood pressure of greater than or equal to 140 mm Hg, diastolic blood pressure was greater than or equal to 90 mm Hg or use of antihypertensive medication. Diabetes was defined as fasting plasma glucose of 126 mg/dL or higher, 2‐hour cholesterol level was defined as 160 mg/dL or higher, or statin use. Low high‐density lipoprotein level was defined a >40 mg/dL in both men and women, or alternatively as <40 mg/dL in men and <50 mg/dL in women (sex‐specific threshold). Hypertriglyceridemia was defined as ≥200 mg/dL. High C‐reactive protein level was defined as 3 to 10 mg/dL, with exclusion of individuals with C‐reactive protein levels above 10 mg/L. Prevalence ratio was adjusted for age, level of education, current health insurance status, field center, national background, nativity, smoking, alcohol use, and physical activity.
