Relation of primary tear size to “Extended” type II and type I aortic dissection. “Extended” DeBakey type II dissection (A) is associated with a larger primary tear with less arch vessel dissection while DeBakey type I dissection (B) is associated with a smaller primary tear and are more likely to have arch vessel dissection and distal progression. For each aortic figure, the arrow on the right denotes true lumen flow while the arrow on the left shows flow entering the tear and into the false lumen.
