  1. Tumor size ≥ 5 cm

  2. Moderate to severe cytologic atypia

  4. Infiltrative margins

 Any 2 of 3 warrant diagnosis of leiomyosarcoma

  1. Moderate to severe cytologic atypia

  2. ≥ 10 mitotic figures/10 HPFs

  3. Tumor cell necrosis
2 of 2 warrant diagnosis of leiomyosarcoma

  1. Moderate to severe cytologic atypia

  2. ≥ 5 mitotic figures/10 HPFs

OR
1 of 1 warrant diagnosis of leiomyosarcoma

  1. Infiltrative margins

Vulva:
Any 2 of 3 warrant diagnosis of low grade leiomyosarcoma and 3 of 3 warrant diagnosis of leiomyosarcoma

  1. Tumor size ≥ 5 cm

  2. ≥ 5 mitotic figures/10 HPFs

  3. Infiltrative margins
