Income was rated on a 6 point scale: 1 = less than $15k, 2 = $15k – $29,999, 3 = $30k – $44,999, 4 = $45k – 59,999, 5 = $60k – $74,999 and 6 = $75k and above. Education was rated on a 7 point scale: 1 = less than 7^th grade, 2 = some high school, 3 = high school graduate, 4 = trade school, 5 = some college, 6 = college graduate and 7 = postgraduate work or degree. CD₂₅, dose (in micrograms) of isoproterenol to increase heart rate by 25 bpm; PD₂₅, dose (in micrograms) of phenylephrine to increase mean arterial pressure by 25mmHg.
