Abbreviations: CVA, cerebrovascular accident; MI, myocardial infarction; PVD, peripheral vascular disease; IABP, intra-aortic balloon pump; LMS, left main stem artery; LV, left ventricle; NYHA, New York Heart Association; and CCS, Canadian Cardiovascular Society.
