High insulin dose

High insulin dose requirement (total daily dose ≥ 1 U/kg/day)

Rapid weight gain with insulin

Polycystic ovary syndrome

Concomitant CV risk factors (central obesity, dyslipidemia, hypertension, family history of CVD) or established CV disease
