Diagnostic criteria based on serum Cr or GFR Diagnostic criteria based on urine output (UO)
Risk Increased serum Cr ≥ 1.5-fold the normal value, or GFR decrease > 25% normal value UO < 0.5 mL/kg/h for 6 hours
Injury Increased serum Cr ≥ 2-fold the normal value, or GFR decrease > 50% normal value UO < 0.5 mL/kg/h for 12 hours
Failure Increased serum Cr ≥ 3-fold the normal value, GFR decrease > 75% normal value, or serum Cr ≥ 4 mg/dL with acute rise ≥ 0.5 mg/dL UO < 0.3 mL/kg/h for 24 h, or anuria for 12 hours
Loss Need for renal replacement therapy for 4 weeks
ESRD Need for dialysis for longer than 3 months
