Fig. 1. Brain magnetic resonance imaging of a patient with perioperative stroke. The patient underwent mitral valve replacement surgery, and right side hemiparesis was found upon arousal from anesthesia. Brain magnetic resonance imaging using diffusion-weighted imaging (DWI) revealed multiple high signal intensities in the bilateral anterior and posterior circulation territories (arrows). The ischemic lesions on DWI suggested multiple embolisms as the underlying mechanism.
