Idiopathic granulomatous mastitis. A 30-year-old woman in the second trimester of pregnancy had a tender lump in the right breast. She had first noticed a lump in the upper outer right breast approximately 2 months previously, and it had increased in size. On physical examination, there was a 10 × 10-cm firm mass in the upper outer quadrant. Biopsies showed acute and chronic granulomatous inflammation with micro-abscesses.
