2. Prechemotherapy platelet count 350 × 109/L or more	1
3. Hemoglobin level less than 100 g/L or use of red cell growth factors	1
4. Prechemotherapy leukocyte count more than 11 × 109 /L	1
5. BMI: 35 kg/m2 or more	1

Three Risk Groups:

▪ Low Risk 0

▪ Intermediate Risk 1–2

▪ High Risk ≥3
