Prior disease was defined as having been diagnosed by a doctor with at least one of the following diseases: diabetes, coronary heart disease, stroke or TIA, hypertension, rheumatic heart disease, tuberculosis, emphysema/bronchitis, asthma, cirrhosis/hepatitis, peptic ulcer, gallbladder disease, kidney disease, rheumatoid arthritis, head injury and cancer.
