Variables with P<0.05 after adjusting for clustering were entered in a model. Backwards elimination was used retaining if p<0.05 for all but ALONE outcomes, which were retained if p<0.10 (because of limited power)

All models were further reduced by excluding 0.6 < OR < 1.5 (weak associations)
