Inclusion and Exclusion Criteria

  • Age ≥ 40 year old

  • No primary care provider

  • Diagnosis lower extremity PAD*

  • Life expectancy under 1 year

  • Able to walk at least 1 block

  • Co-morbidities which limit physical activity to a severe degree+

  • Are sedentary (defined as self-reported PA of < 150 minutes per week)

  • Signs of critical limb ischemia and/or planned revascularization in the next 12-months

  • Medical clearance from primary care provider

  • Recent or current enrollment in formal exercise program

  • Competent to give informed consent

  • Recent CVD events (< 3 months) including stroke/TIA, MI, UA, PCI/CABG, or severe valve disease, congenital HD, complex arrhythmias (untreated), NYHA class III–IV HF

  • Must be a regular email user

  • Current substance abuse

  • Have access to a computer with internet connection,

  • a USB port and Windows operating platform

  • Psychiatric disorder#

  • Pregnancy

  • Non-English speaking

*

PAD = peripheral arterial disease with a documented ankle brachial index of < 0.9 or documentation via imaging (MRI or CT scan, or angiogram);

inability to walk one city block;
