Pearls and Pitfalls

  • Outside-in repair is best suited for tears of the body and anterior horn of the meniscus. Tears to the posterior horn may be better suited for an inside-out or all-inside repair.

  • Capture only 1 leaflet of the meniscus with each pass of the crossing mattress suture.

  • Before tying, gently pull on 1 limb of the suture and shorten the other limb so that if the suture was partially torn during needle passing, the partial tear is brought outside of the knee. In this way, the knot is not compromised.

  • Tie down the sutures in the order they were placed with the crossing suture first.
