Patient positioned in modified beach chair position. A 4-cm incision is made perpendicular to the long axis of the right clavicle approximately 2 cm medial to the acromioclavicular (AC) joint. The deltotrapezial fascia is carefully incised in line with the clavicle, exposing the fracture site. The soft tissues medial and lateral to the coracoid should be bluntly dissected to allow for uncomplicated passage of graft and sutures.
