The propensity score model includes age, race, sex, income, education, diabetes mellitus, smoking, hyperlipidemia, hypertension, obesity, coronary heart disease, stroke, heart failure, end‐stage renal disease, disease severity, any‐cause office visit within 1 year before diagnosis, and self‐rated health. The modeling strategy is described by Brookhart et al16.
