Advantages • Restore joint congruency biologically • Delay metal solution in young patients • Concomitant surgeries can be performed simultaneously • Fresh allografting keeps the chondral layer with viable chondrocytes

Limitations • Fresh allografting has considerable logistical limitations • Demanding technique • Non-weight-bearing condition for 3 mo postoperatively • Only a 10-mm-thick osteochondral allograft must be transplanted
