Model 2 adjusted for age, race (white or non-white), family history of cardiovascular disease (yes or no), family history of cancer (yes or no), physical activity (<3, 3-8.9, 9-17.9, 18-26.9, or >27 MET-h/week), alcohol consumption (0, 0.1-4.9, 5-9.9, 10-14.9, or ≥15 g/day), total energy intake (fifths), smoking status (never, ever, 1-14, 15-24, or ≥25 cigarettes/day), and Alternate Healthy Eating Index (fifths). Additionally, excluded participants with lean body mass below 2.5th centile. For exclusion analyses, height adjusted lean body mass was used after regression out of variation due to height.
