Good physical health = no multimorbidity and no mild disability; fair physical health = multimorbidity, disability, or both; poor physical health = multimorbidity and severe disability; good cognitive health = Mini-Mental State Examination (MMSE) ≥ 26; fair or poor cognitive health = MMSE ≤ 25.
