Two Hohmann retractors protect the tibial head. With a rasp, the superficial portion of the medial collateral ligament is carefully separated. Then, the first Kirschner wire (K-wire) is drilled obliquely in the proximal tibia. The entry point of the first K-wire is just below the most inferior fibers of the patellar tendon.
