Estimates adjusted for age, sex, race/ethnicity (black, white, Hispanic, Asian, or other), ZIP code–level income derived from national census data, the presence of full Medicaid benefits (defined in the first month of Medicare eligibility in the measurement year), and indicator variables for hospital referral regions.
