What have you learned that might help other caregivers caring for their loved one within that first week of coming home from the hospital?
2-weeks post-discharge	How have you and your [relative/friend] been since I saw you last week? Tell me about what this week has been like for you and your [relative/friend]. Has anything changed since we last spoke in regard to … Have some things become easier in caring for your [relative/friend] this week? If yes, what? Why was it easier? Was anything harder in caring for your [relative/friend] this week? If yes, what? Why was it harder, what exactly about that was harder?
