For model 1, the study population was categorized by fertility status as fertile (reference), subfertile, and IVF (limited to autologous-fresh cycles only); for model 2, the study population was limited to IVF-conceived pregnancies as autologous-fresh (reference), autologous-thawed, donor-fresh, donor-thawed
