The opposite directional vector forces are always in balance in the normal patient (red vertical line). The posterior vectors require a firm uterosacral ligament (USL) insertion point. If USL is loose, the posterior vectors weaken and the system loses balance, moving to the left of the red vertical line. PCM compensates by contracting forwards.
