Lymphatic drainage to the neck is unilateral in a 55-year-old male patient with a T1 primary tumour localized on the right side of the tongue (on the left) and bilateral in a 72-year old female patient with a T1 midline tongue carcinoma (on the right). Note that in both cases the sternocleidomastoid muscle, as depicted on volume rendering and cross-sectional SPECT/CT, is an excellent landmark to anatomically refer the location of sentinel lymph nodes in relation to lymphatic basin and surgical neck level
