A 59-year-old female with hepatitis B virus-related cirrhosis and hepatocellular carcinoma (HCC) exhibiting an intermediate enhancement pattern. A 2.7-cm tumor in segment 7 of the liver shows low-signal intensity on precontrast T1-weighted imaging (a). The center of the lesion demonstrates lower arterial enhancement compared to the peripheral portion (b) on gadoxetate-enhanced MRI. On pathologic examination, the HCC shows a nodule-in-nodule pattern (c, Sirius red stain; d, hematoxylin-eosin [H-E] stain). On histopathologic analyses, the outer nodule shows tumor trabeculae with a thickness of < 10 tumor cells and no macrotrabecular pattern (e, H-E stain), a capillary-like microvascular pattern on immunostaining for CD34 (f), and no carbonic anhydrase IX (CAIX) expression (g). Conversely, the inner nodule exhibits a macrotrabecular pattern (h, H-E stain), a sinusoid-like microvascular pattern on immunostaining for CD34 (i), and is positive for CAIX (j).
