The Community Transformation Grant (CTG) program, funded from 2011–2014, helped communities design and carry out local programs to prevent chronic diseases such as obesity, diabetes, and heart disease. The Communities Putting Prevention to Work (CPPW) program, funded from 2010–2012, supported 50 communities working to reduce obesity and tobacco use.
