  • Right heart catheterisation with assessment of cardiac index, right and left heart pressures

  • Heart Failure Survival Score

  • Seattle Heart Failure Model

  • Comorbidity profile, including respiratory status, liver and kidney function

  • Current or prior diagnosis of cancer

  • Current psychological status and prior psychological problems

  • Current HF treatment

  • Inotrope dependency

  • Repeated hospitalisations for congestion

  • NYHA class III–IV and

  • Intolerance to HF medication or

  • Hypotension or

  • Anaemia or

  • Deterioration of renal function or

  • Repeated HF hospitalisations
