Criteria for metabolic risk factors: hemoglobin A1c >48 mmol/L, systolic blood pressure >140 mm Hg, body mass index >30 kg/m2, HDL‐cholesterol <1.0 mmol/L for women and <0.9 mmol/L for men, triglycerides >1.7 mmol/L, and presence of micro‐ or macroalbuminuria.
