Interventions Intervention group: received telephone follow‐up calls at 3 months and 6 months from time of randomisation. Telephone follow‐up call included evaluation of medication adherence based on pharmacy refill history, as well as continuing stroke education and reassessment of stroke prevention goals with the participant. Recommendations for medication therapy and relevant clinical studies or laboratories were communicated to the primary care provider and/or stroke provider when appropriate
